The form directs you to provide a summary of what you’ve experienced. 

To protect you from repeating distressing details, please keep the information you share on the form brief.

We know it can be difficult to go over the details of what has happened, and we do not want you to experience any unnecessary discomfort.  

If you submit a form requesting Support from an Adviser, an adviser will contact you to discuss any appropriate support and reporting options with you. Following your conversation with a member of Student Wellbeing Services (or HR for staff submitting a report), if you choose to make a formal report for investigation you will be connected to the relevant team to take your report forward. As part of the reporting process, you will have an opportunity to share what's happened in a supported manner.
