Report

Please read the information below before completing and submitting your form:

 
If you see or experience harassment, discrimination, hate crime, assault, bullying, sexual violence, spiking or domestic abuse, you can submit a named or an anonymous report. Submitting a named report will mean that a member of Student Wellbeing Services contacts you to offer an appointment, to discuss your options. 
 
 
In an emergency 
 
For emergencies and urgent help, please call 999.

For non-urgent medical attention – contact your GP or call 111 
 
For sexual violence that has happened within the last 7 days, your local Sexual Assault Referral Centre can take forensic evidence. You do not need to make a decision about reporting to the police (forensic evidence can be stored for up to 7 years). They can also help with emergency contraception and sexual health testing.  
 
Do not submit a Report and Support form until after you have received emergency help. 
 
I want to request that the University takes formal action 

Submitting a form on Report and Support does not directly initiate a formal disciplinary investigation. It will connect you to Student Wellbeing Services (or HR for staff-related reports) who can discuss reporting and support options, and support you to make a formal disciplinary report if you want to do this.  
 
Confidentiality

Your submitted form is treated confidentially within Student Wellbeing Services (or HR for staff-related forms).
Please note that we cannot guarantee confidentiality for you and/or others named in a report if there are risk concerns.
